The role features a M-F 8:00a-5:00p schedule with rotating weekend and overnight call, emphasizing health assessments, care plans, orders for labs, documentation, and inter-professional communication.

Qualifications

• Master's degree in Nursing or Physician Assistant required.
• Experience with geriatric patients preferred; training available for new grads.

Responsibilities

• Provide patient care to seniors in nursing homes, assisted living, and memory care settings.
• Perform health assessments and follow mutually agreed treatment plans.
• Order labs and diagnostic tests; follow up on results.
• Document care accurately and communicate with families and other health professionals.
• Serve as a preceptor and participate in orientation and quality improvement projects.
• Collaborate with geriatricians, nurses, and staff to deliver comprehensive care.
