Job summary

Carle Health in Champaign-Urbana, Illinois, seeks an advanced practice provider to join our geriatrics team. You will work with geriatricians, APPs, nurses and home staff to deliver care across nursing homes, assisted living, and memory care in the Champaign-Urbana area.

Typical hours are Monday–Friday, 8:00am–5:00pm with rotating paid weekend and weekday overnight call. You will order labs, follow results, document care, and coordinate with families and referral networks.

Qualifications

• Master's degree in Nursing or Physician Assistant.
• Background working with Geriatric patients is ideal, but team is willing to train New Grads.

Responsibilities

• Provide patient care to senior residents in nursing homes, assisted living and memory care within Champaign-Urbana area.
• Orders labs & diagnostic testing, follows up on results.
• Ensures documentation of care is complete and accurate; communicates with other health professionals, families, and referral agencies.
• Acts as a preceptor and/or participates in orientation of new employees; participates in quality improvement projects.
