Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with primary care or internal medicine experience, preferably geriatrics-focused, to join our team serving seniors in long-term care settings. You will improve outcomes by collaborating with providers, case managers, and facility partners within nursing homes or assisted living communities.

You’ll exercise independent judgment, treat acute and chronic conditions, and work with a supervising physician to implement our national care model

Qualifications

• APRN/NP or PA license is required.
• Masters degree or higher.
• 3 years of geriatric clinical nursing or rehab experience.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor patient response and adjust treatment plans.
• Collaborate with RN case managers in an integrated care team.
