Job summary

TruHealth is seeking an advanced practice provider (NP or PA) to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver personalized, coordinated health care in nursing homes and assisted living communities.

The role requires an APRN or PA license and a degree from an accredited program, with 3 years in geriatric clinical nursing or rehab. Expect strong benefits, and growth opportunities within a nationwide model of care.

Qualifications

APRN or PA licensure required.
Degree from an accredited APRN or PA program.
3 years’ experience in clinical nursing or rehab in geriatric populations.
Experience with Electronic Health Records.
Proficiency with MS Office.

Responsibilities

Assess patients’ medical and mental health needs and create a plan of care.
Ensure compliance with local, state, and federal regulations for clinical services.
Prescribe medications and order labs, diagnostics and consultations.
Monitor patient compliance and response to treatment.
Collaborate with RN case managers as part of an integrated care team.
