Job summary

TruHealth is seeking an advanced practice provider (NP/ANP or PA) to deliver primary care to residents in nursing homes and assisted living. You will exercise independent judgment, manage acute and chronic conditions, and collaborate with physicians, facility staff and health plan leadership to implement treatment protocols.

Qualifications

APRN or PA license is required.
Degree from an accredited APRN or PA program.
3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
Electronic Health Records experience.
Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

Assess patients’ medical and mental health needs and provide a plan of care.
Ensure compliance with local, state, and federal regulations related to clinical services.
Prescribe medications and order labs, diagnostic procedures and consultations.
Monitor patients’ progress and modify treatment plans as needed.
