Job summary

TruHealth in Greenfield, IN is seeking an advanced practice provider (NP or PA) with geriatric primary care or internal medicine experience to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers within nursing homes or assisted living facilities enrolled in Medicare Advantage plans.

The role emphasizes independent clinical judgment, coordination with supervising physicians, and adherence to all regulations.

Qualifications

• APRN/PA license required
• Master's degree or higher
• 3 years' experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• MS Office applications experience

Responsibilities

• Assess patients' medical and mental health needs and create a proactive plan of care
• Ensure compliance with local, state, and federal regulations
• Prescribe medications and order labs, diagnostics and consults
• Monitor treatment response and adjust plans as needed
• Collaborate with RN case managers as part of an integrated care team
