Job summary

Seniority Healthcare is seeking a part-time Nurse Practitioner or Physician Assistant to join our team in New City, NY for 22 hours a week. You will provide comprehensive care to a high-need, high-risk geriatric population, working 2 days onsite at an assisted living facility and 3 days remotely for prescription refills, lab reviews, and patient communication.

The ideal candidate has experience in Internal Medicine, Family Medicine, or Geriatrics and can work independently while collaborating

Qualifications

• Completion of an accredited NP or PA program.
• At least one year of experience in geriatrics, internal medicine, or family practice.
• Strong communication, active listening, and analytical skills.
• Compassionate and committed to providing patient-centered care.
• Motivated to serve a geriatric population with complex care needs.

Responsibilities

• Conduct thorough physical exams and wellness assessments for seniors.
• Follow up on care plans to ensure the best possible outcomes.
• Propose and manage treatment plans for chronic and acute conditions.
• Maintain accurate patient records and manage appointments effectively.
• Educate patients on preventative care, treatments, and healthy lifestyle choices.
• Collaborate with a supportive team dedicated to holistic care.
