Job summary

Community Physicians is seeking a Board-certified NP or PA to join our outpatient geriatric care team. You’ll co-manage a defined panel alongside a lead physician, collaborating with a dedicated team to deliver prevention, chronic disease management, and goal-oriented care in a clinic setting.

You’ll review medications, order labs, and ensure accurate records while exercising independent clinical judgment in a supportive environment.

Qualifications

• Board-certified NP or PA with geriatrics interest.
• Comfortable in outpatient clinical settings.
• Strong communication with patients and families.

Responsibilities

• Co-manage a defined panel of geriatric patients with a lead physician.
• Collaborate with a multidisciplinary team to provide high-quality care.
• Perform preventative screenings and discuss long-term goals of care.
• Review and update medication lists and order labs and imaging as needed.
• Maintain accurate patient records and ensure continuity of care.
• Demonstrate independent clinical decision-making in a fast-paced setting.
• Support care across post-acute settings such as AL/IL and SNFs.
