Job summary

Longevity Health in Chicago, IL is seeking a Nurse Practitioner or Physician Assistant to provide collaborative primary care for a panel of nursing home residents. You will perform comprehensive assessments, develop individualized care plans, and participate in care-team meetings with families and staff.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• Master of Science in Nursing as an NP or Master’s from an accredited program.
• Experience in nursing home environment strongly desired.
• Computer/Technology proficiency required.
• Ability to engage and communicate effectively with physicians, members, and family.
• Self-directed but functions well in a team environment.
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing for chronically ill.
• Demonstrated organizational and prioritization skills.
• Active state licensure as NP or PA with national board certification (AANP/ANCC/AACN or NCCPA).
• DEA License required.
• Active NPI and Medicare provider number.
• Eligible to participate in Medicare as a provider.
• Valid state driver’s license with good record and insurance.
• Auto liability insurance with minimum limits as required.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP activities and advanced care planning to reduce hospitalizations.
• Manages authorization, Part A/B services, medication reviews, and quality gap closures.
• Serves as a trusted advisor to members, families, and facility staff.
• Communicates with PCPs, families, and care teams about care plans and changes.
• Supports market goals and balances member caseloads.
• Promotes Longevity Health with stakeholders.
• Functions autonomously and remains accountable for practice.
• Builds meaningful relationships in nursing home environments.
• Maintains member-centric, high-quality care and service.
• Performs other related duties as assigned.
