Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long-term care residents. You will perform comprehensive assessments, develop individualized care plans, and work with a dedicated clinical leadership team to ensure high-quality, individualized care.

You will participate in nursing facility care team meetings, maintain frequent communications with families and PCPs, and uphold the goals and needs of each member in a nursing home setting.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN/NP program or Master's in Nursing from an accredited program.
• Experience working in the nursing home environment is strongly desired.
• Active state NP or PA license and DEA license.
• Must have an active NPI and Medicare provider number.

Responsibilities

• Provide collaborative primary care to a panel of members.
• Conduct comprehensive assessments and develop individualized care plans.
• Participate in nursing facility care team meetings.
• Communicate with members' families and PCPs regularly.
• Support quality measures including HEDIS and STARs.
• Promote patient-centered care and autonomy.
