Qualifications

• Current CDS and DEA licenses.
• 2 years of experience as a Nurse Practitioner.
• Family or Adult NP certification preferred.

Responsibilities

• Provide specialized geriatric assessments and follow-ups in outpatient settings.
• Provide inpatient geriatric consultations.
• Deliver primary geriatric care in sub-acute, assisted living and long-term care settings.
• Review medications, labs, and diagnostic tests.
• Participate in on-call coverage within the department.
• Provide patient-centered care emphasizing education, health management, and prevention.
• Assist patients and families in identifying goals of care and advanced care planning.
