Job summary
CareMed Health Solutions seeks a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities in Newark, OH. The NP will work within an interdisciplinary team to manage chronic conditions, order tests, and collaborate with primary care physicians to ensure high-quality resident care.
The ideal candidate holds an Ohio NP license, board certification, DEA licensure, and EMR proficiency, with strong patient-focused care.
Qualifications
- Current, unrestricted NP license in Ohio.
- Board-certified NP with DEA licensure.
- Experience in long-term care and EMR proficiency.
Responsibilities
- Provide primary and preventive care for long-term care residents.
- Manage residents with acute changes in condition.
- Coordinate care through interdisciplinary teams and with primary physicians.
- Prescribe medications in accordance with OH law.
- Order and follow up diagnostic tests and treatments.
- Communicate with nursing home staff about changes in condition.
- Contribute to care planning and documentation.
