Job summary

Trio Health Partners is seeking an In-House Nurse Practitioner to provide internal medicine services within a Skilled Nursing Facility in Illinois. You will lead primary care for residents, build relationships with families and staff, and contribute to clinical leadership in our geriatric practice.

Qualifications

• Active Illinois NP license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience preferred, with focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsibilities

• Conducting new admission evaluations and monthly stability visits for long-term residents
• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care
