Job summary

TRIO HEALTH PARTNERS is seeking an In-House Nurse Practitioner specializing in Infectious Disease to lead care in our geriatric facilities. The NP will evaluate residents, prescribe treatments, and contribute to your facility's clinical leadership while coordinating with staff and families.

We offer flexible full-time schedules, no nights or weekends, a monthly bonus program, and comprehensive health benefits, 401(k), and CME support. Illinois NP license required.

Qualifications

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsibilities

• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care
