Job summary

Trio Health Partners is seeking an In-House Nurse Practitioner to serve as a primary provider of internal medicine services within a Skilled Nursing Facility. You will build strong relationships with residents and families while integrating into the clinical leadership team.

Responsibilities include admissions evaluations, comprehensive exams, prescribing medications, and coordinating care plans to prevent readmissions, with a focus on geriatric and long-term care populations.

Qualifications

• Active Illinois Nurse Practitioner license (unencumbered).
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience with focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Job Requirements:
