Job summary

Trio Health Partners is seeking an In-House Nurse Practitioner to serve as the primary internal medicine provider within a Skilled Nursing Facility. You will build lasting relationships with residents, families, staff, and providers while contributing to the clinical leadership team.

The role offers a flexible full-time schedule with no nights or weekends, monthly bonus opportunities, and comprehensive benefits including health, 401(k), PTO, CME/stipends, and tail insurance.

Qualifications

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in primary care and preventive medicine.
• Geriatric or adult medicine experience in skilled nursing/long-term care settings.
• Ability to engage and communicate effectively with elderly patients and families.
• Able to meet a minimum of 12 patient visits per day; bonuses for surpassing target.
• Knowledge of Medicare/Medicaid and Health Plan benefit structures is a plus.

Responsibilities

• Conduct new admission evaluations and monthly visits for residents.
• Perform comprehensive physical exams and assess changes in patient conditions.
• Prescribe medications and treatment plans as needed.
• Develop patient care policies and coordinate referrals to specialists.
• Coordinate advanced care planning and discharge planning.
• Ensure timely communication and documentation of assessments and plans.
• Support the provider-patient relationship and address gaps in care.

As an In-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). Job Requirements:

• Active Illinois Nurse Practitioner License (unencumbered).

Key Responsibilities: As the In-House Nurse Practitioner, you will provide essential care and leadership within the facility, including:

• Prescribing medications and treatments as needed

• Developing patient care policies and facilitating referrals to specialists

• Coordinating advanced care planning and discharge visits

• Supporting the physician-patient relationship and addressing gaps in care

Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.

Salary & Compensation:

• Annual Pay: $105,000 - $130,000 based on experience

• Monthly Bonus Opportunities: Monthly performance-based bonuses
