Job summary

Qualifications
- Active NP with geriatrics focus.
- At least 2 years in geriatric care (nursing home/long-term care/geriatric clinic).
- Strong communication and collaboration skills.
Responsibilities
- Conduct comprehensive physical examinations for geriatric patients.
- Develop individualized care plans addressing medical, physical, and psychological needs.
- Prescribe medications, order/interpret tests, perform procedures within scope.
- Collaborate with physicians, nurses, social workers, and therapists for coordinated care.
- Educate residents and families about health conditions and treatments.
- Advocate for residents' well-being and appropriate care.
- Document care accurately in the EHR.
