Qualifications

• APRN or PA license is required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or fee-for-service experience.
• Experience with Electronic Health Records.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and modify care plans as needed.
