Qualifications

• Active Pennsylvania NP license required.
• 1+ year of experience in geriatrics and documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsibilities

• Conduct on-site visits to skilled nursing facilities to provide high-quality preventative care to residents.
• Partner with our scribe team to ensure accurate and timely documentation.
• Collaborate with our clinical directors, regional managers, and supervisors to provide patient-centered care.
• Develop and maintain strong relationships with facility staff and patients.
• Maintain accurate and up-to-date patient records.
