Job summary

Carease Health - PA is seeking an experienced and compassionate Adult Nurse Practitioner to join our preventative healthcare team. This hybrid role combines on-site visits to skilled nursing facilities with remote work from home, supported by a dedicated scribe team for efficient documentation.

You will perform comprehensive assessments, develop treatment plans, collaborate with facility staff, and use remote monitoring to track patient health while maintaining a patient-centered approach.

Qualifications

• Active NP license in PA.
• 1+ year of geriatric care experience.

Responsibilities

• Provide proactive and collaborative care to residents in skilled nursing facilities through on-site visits and remote monitoring
• Conduct comprehensive assessments, develop treatment plans, and implement interventions to improve patient outcomes
• Work closely with facility staff, including physicians, nurses, and caregivers, to ensure seamless delivery of care
• Utilize remote patient monitoring technologies to track patient vital signs and detect potential abnormalities
• Participate in regular case conferences and quality improvement initiatives to enhance patient outcomes and reduce hospitalizations
• Document patient information accurately and efficiently, utilizing a scribe team for support
