Job summary

Senior Community Care of Colorado PACE seeks a compassionate Nurse Practitioner to deliver primary care to our PACE participants across Eckert and Montrose sites. The role involves assessments, chronic disease management, and collaboration with an interdisciplinary team to support home and campus-based care.

Licensed in Colorado as an NP with prescriptive authority, you should have geriatric experience and participate in an on-call rotation.

Qualifications

• RN license in the state of employment.
• Certified as a Nurse Practitioner with prescriptive authority within one year.
• Geriatric NP certification preferred.
• Experience in geriatric care and frail elderly populations.

Responsibilities

• Provide primary care to PACE participants under Medical Director supervision.
• Perform comprehensive H&Ps for new enrollees and semi-annual reassessments.
• Develop and implement care plans; coordinate with the IDT.
• Manage chronic diseases and acute illnesses in clinic or home.
• Participate in on-call rotation and perform other duties as required.
