Job summary

BoldAge PACE at Acutecare Health System delivers primary care and continuous healthcare services to PACE participants. You will assess, plan, and provide care aligned with program standards, collaborating closely with the clinical team to develop and execute comprehensive care plans.

The role focuses on individualized evaluation, medication safety, and active participation in IDT meetings, with on-call coverage to ensure seamless care for participants.

Qualifications

• MD/DO/NP/APRN/PA credentials required or eligible per NJ rules.
• New Jersey State Board certification or eligibility for advanced practice.
• Basic life support (BLS) within 90 days of employment.
• Experience delivering direct patient care is preferred.
• Familiarity working with frail elderly populations is a plus.

Responsibilities

• Deliver primary care and continuous healthcare services per program standards.
• Assess, plan, and provide care with the clinical team.
• Develop care plans, reassess, and coordinate with specialists as needed.
• Monitor medications and reconcile with pharmacology vendor.
• Participate in IDT meetings and discharge planning as required.
• Support 24/7 on-call coverage and ensure safe transitions.

Education

MD, DO, NP, APRN, or PA

Board Certified in Geriatrics preferred

BLS within 90 days

2 years direct patient care

1 year elderly care experience
