Job summary

360care is seeking a Nurse Practitioner to deliver direct patient care in nursing homes and long-term care facilities. You will perform geriatric assessments, medical histories, exams, and treatment plans, with documentation aligned to 360care standards.

Collaborate with physicians and care teams to ensure high-quality patient care, participate in rounds, and support risk and quality programs as needed.

Qualifications

• RN licensure and Nurse Practitioner Certification in the state of practice

Responsibilities

• Provide direct patient care in nursing homes or long-term care facilities
• Perform geriatric assessments, medical history, physical exams, and treatment planning
• Document progress notes and ensure compliant documentation
• Collaborate with physicians and care teams to ensure quality patient care
