Job summary
CareMed Health Solutions is seeking a Nurse Practitioner to provide medical services to residents in long-term care facilities. You will deliver primary and preventive care, manage acute changes, and participate in care planning within an interdisciplinary team.

Qualifications
- OH license must be current and unrestricted.
- Board certification required.
- DEA licensure required for prescribing controlled substances.
- Experience in long-term care settings preferred.
- Ability to work with EMR and interdisciplinary teams.
Responsibilities
- Provide primary and preventive care for long-term care patients.
- Manage residents with acute changes in condition.
- Perform advanced care planning and chronic care management.
- Prescribe medications, including controlled substances in OH.
- Order and follow up with treatments and diagnostics.
- Communicate with nursing home staff about condition changes.
- Collaborate with primary care physician.
- Support care plan development.
