Job summary

Vitae Healthis seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.

The role emphasizes collaboration with the clinical team, physicians, and nursing staff to coordinate comprehensive care while maintaining HIPAA compliance and ethical standards.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in respective state (LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with the Vitae clinical team, attending physician, facility nursing staff, social workers, and rehabilitation teams.
• Participate in care plan meetings and interdisciplinary team discussions.
• Maintain accurate and timely clinical documentation complying with regulations.
• Provide crisis intervention and behavioral management strategies when needed.
• Educate staff and families on mental health conditions and effective communication strategies.
• Ensure HIPAA compliance and ethical standards of practice.
