The position requires a Master’s degree and an active state license, with 2 years of clinical experience preferred. Expect collaborative care, unique schedule, and strong documentation in a team setting.

Qualifikationen

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in the respective state (LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience in geriatric, long-term care, or medical settings preferred.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of SNF mental health conditions (depression, anxiety, dementia).
• Proficiency in evidence-based approaches (CBT, trauma-informed care, grief counseling).
• Ability to work collaboratively within an interdisciplinary team.

Aufgaben

• Conduct psychosocial assessments for residents at admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, dementia, and adjustment to illness.
• Collaborate with care team to coordinate services.
• Maintain accurate clinical documentation per regulations.
• Provide crisis intervention and behavioral management when needed.
