Job summary

Vitae Health Systems is seeking a Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.

The role collaborates with an interdisciplinary team to coordinate care, document thoroughly, and deliver evidence-based therapies in a compassionate, HIPAA-compliant environment.

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with the Vitae clinical team, the patient's attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care.
• Participate in care plan meetings and interdisciplinary team discussions.
• Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations.
• Provide crisis intervention and behavioral management strategies when needed.
• Educate staff and families on mental health conditions and effective communication strategies.
• Ensure compliance with HIPAA and ethical standards of practice.
