Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well-being and adjustment to medical changes.

The role requires a Master’s degree, active license, and 2+ years of geriatric clinical experience. You’ll work within an interdisciplinary team, documenting care and addressing depression, anxiety, and dementia-related

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field required.
• Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2+ years clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of mood disorders, dementia, and adjustment issues common in SNF populations.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with interdisciplinary team to coordinate care and participate in care plan meetings.
• Maintain accurate clinical documentation in line with regulations.
• Provide crisis intervention and behavioral management when needed.
• Educate staff and families on mental health and effective communication strategies.
