Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions to support residents’ well-being and adjustment.

The role involves collaborating with interdisciplinary teams, maintaining HIPAA-compliant documentation, and providing crisis intervention as needed. Requires a Master’s degree and an active license in the relevant state.

Qualifications

• Conduct psychosocial assessments for residents at admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, dementia, and adjustment to illness or disability.
