Job summary

Behavioral Health Solutions, Inc. is seeking independently licensed therapists (LICSW, LMFT, LPC) to join our SNF Therapy team in Colorado, serving the geriatric population in skilled nursing and long-term care facilities.

The role offers base salary plus incentive opportunities with total earnings potential of $70,000 - $100,000+ annually. We provide stability, support, and flexible scheduling, along with generous PTO, CEU allowances, and access to AI note-taking tools to streamline

Qualifications

• Master of Social Work, Counseling, or other eligible master's degree required with Colorado licensure as LCSW/LMFT/LPC.
• NPI required; Medicaid/Medicare credentialing is a plus.
• Vaccinations or exemptions may be required at some facilities.
• TB Test and CPR/AED/First Aid Certification required.

Responsibilities

• Completes diagnostic evaluations, diagnosis, psychotherapy, treatment planning, and consulting in-person at partner SNF/LTC facilities.
• Collaborates with BHS psychiatrists, psychologists, care coordinators, and facility staff for holistic patient care.
• Completes documentation in a timely fashion remotely in the EMR (Athena I).
• Ensures compliance with agency policies and procedures.
