Job summary

Behavioral Health Solutions is seeking independently licensed therapists (LICSW, LMFT, LPC) to join our SNF Therapy team in Colorado, serving geriatric residents in skilled nursing and long-term care facilities. You will conduct evaluations, psychotherapy, and treatment planning, collaborate with psychiatrists and care staff, and document in Athena EMR.

Qualifications

• Master of Social Work or equivalent MS degree and Colorado licensure (LCSW/LMFT/LPC).
• NPI number required.
• Credentialed with Medicaid/Medicare.
• Vaccinations up-to-date or approved exemptions.
• TB test and CPR/AED/First Aid certification.

Responsibilities

• Conduct diagnostic evaluations, psychotherapy, treatment planning and consulting in SNF/LTC facilities.
• Collaborate with BHS psychiatrists, psychologists, care coordinators, and facility staff.
• Complete documentation remotely in Athena EMR in a timely manner.
• Comply with agency policies and procedures.
