Behavioral Health Solutions in Vermont is seeking qualified Independently Licensed Therapists (LCSW, LMFT, or LPC) for its Long Term Care and Nursing Facility Therapy team. Candidates will provide comprehensive behavioral health services to geriatric patients.

Qualifications

• Master's degree and Utah licensure as LICSW, LMFT, or LPC required.
• NPI (National Provider Identifier) is mandatory.
• Credentialing with Medicaid/Medicare is a plus.

Responsibilities

• Completes diagnostic evaluations and treatment planning in-person.
• Collaborates with multidisciplinary teams for patient care.
• Documents in EMR and complies with procedures.
