Job summary

Trio Health Partners is seeking a Mental Health Therapist (Psychotherapist) to provide primary in-house psychotherapy at a Skilled Nursing Facility. You will build relationships with residents, families, and staff while delivering integrated behavioral and healthcare services.

You will hold an Illinois clinical license (LCSW/LCPC/Clinical Psychologist), manage a caseload with 5+ therapy hours daily, and collaborate with an interdisciplinary team to meet residents’ emotional and psychological

Qualifications

• Licensure must be Illinois-licensed (LCSW, LCPC, or Clinical Psychologist).
• Experience with geriatric/long-term care populations preferred.
• Strong communication, multitasking, and patient-focused care required.

Responsibilities

• Conduct initial assessments and provide ongoing psychotherapy for residents.
• Manage caseload with a minimum of 5 therapy hours daily.
• Collaborate with an interdisciplinary team to address emotional, social, and psychological needs of residents.
• Ensure documentation compliance for Medicare and commercial plans.
