Job summary

Trio Health Partners in Chicago area is seeking a Mental Health Therapist (Psychotherapist) to provide in-house psychotherapy at a skilled nursing facility. You will build relationships with residents, families, and staff while integrating behavioral and physical healthcare.

Qualifications

• Illinois clinical license required (LCSW, LCPC or Clinical Psychologist).
• Geriatric or long-term care experience preferred.
• NPI number required (assistance available).
• Medicare/Medicaid knowledge beneficial.

Responsibilities

• Conduct initial assessments and provide ongoing psychotherapy.
• Manage caseload with at least 5 therapy hours daily.
• Collaborate with interdisciplinary team to meet residents' needs.
• Ensure documentation complies with Medicare and insurers.
