Job summary

GeriHeal P.C. is seeking an experienced Medical Doctor / Supervising Physician to provide clinical leadership and medical oversight across Assisted Living Facilities, Skilled Nursing Facilities, and related care settings.

The role supports physicians and APPs, guiding clinical decision-making and coordinating care for older adults in post-acute and long-term care environments. The ideal candidate will be an Internal Medicine or Geriatric Medicine physician with leadership experience, strong

Qualifications

• MD or DO with an active, unrestricted medical license in the state of practice.
• Board certification or eligibility in Internal Medicine, Family Medicine, Geriatric Medicine, Emergency Medicine, Pediatrics, or related specialties; post-acute care experience desirable.
• Experience supervising physicians, APPs, or multidisciplinary clinical teams.
• Demonstrated experience in ALF/SNF/geriatric settings; strong leadership and organizational skills.

Responsibilities

• Provide clinical leadership and physician-level oversight for medical providers in ALF/SNF settings.
• Supervise and support physicians and APPs in accordance with laws and protocols.
• Offer clinical consultation and guidance for complex cases.
• Promote consistency in clinical practices, documentation, care planning, and quality standards.
• Support provider development and adherence to standards of care; oversee medical services in ALF/SNF environments.
• Evaluate and manage acute, chronic, and complex conditions; guide treatment plans and medication management.
• Collaborate with facility administrators, nursing, rehabilitation, and other healthcare professionals.
• Review clinical documentation, labs, and treatment plans; promote evidence-based practices and quality improvement.
• Strengthen communication and continuity of care across hospitals, ALFs, SNFs, and outpatient providers.
