Job summary

Carease Health is seeking experienced physicians to join our national medical provider services organization as Medical Directors overseeing a portfolio of Skilled Nursing Facilities. We focus on geriatric, post-acute, and long-term care medicine, promoting wellness, quality of life, early issue identification, and reducing avoidable hospitalizations.

Medical Directors will lead clinical teams regionally, mentor NPs, collaborate with facility leadership, develop evidence-based guidelines,

Qualifications

• MD or DO from an accredited medical school.
• Board certified or board eligible in Internal Medicine, Family Medicine, Geriatric Medicine, or related specialty.
• Active, unrestricted medical license in applicable state(s).
• Experience caring for older adults and medically complex patients.
• Fellowship in Geriatric Medicine preferred; SNF Medical Director experience ideal.

Responsibilities

• Provide clinical leadership across a designated group of SNFs.
• Conduct rounds with NPs and provide mentorship.
• Serve as a clinical resource for complex patients and goals-of-care discussions.
• Promote proactive identification and management to reduce hospitalizations.
• Collaborate with nursing, rehab, and interdisciplinary teams to promote wellness.
• Develop and implement evidence-based guidelines and care pathways.
• Monitor quality metrics and lead performance improvement initiatives.
• Build relationships with facility and organizational leadership.
• Foster a culture of clinical excellence across the network.
• Deliver cost-effective, high-value care while reducing Medicare costs.
