Job summary

Qualifications

• Graduate of an accredited school of vocational/practical nursing.
• Active state LVN license.
• One year of LVN experience in a clinical care setting.
• Home health experience preferred.
• Homecare Homebase platform experience is a plus.

Responsibilities

• Assist in identifying patient needs and plan of care under RN supervision.
• Deliver nursing care as ordered and educate patients and caregivers.
• Ensure infection control and documentation are maintained.
• Maintain timely communication with patient, family, and healthcare team.
