Home Care Providers of Texas is seeking a PRN Licensed Vocational Nurse (LVN) to join our team in the SW Houston, TX area. You will perform skilled nursing visits according to the patient’s plan of care and physicians' orders, delivering compassionate, evidence-based care in patients' homes.

Requirements include active Texas LVN license, ALF experience, CPR certification, and geriatric home health background.

Qualificações

• Active Texas LVN license is required.
• Experience in Assisted Living Facilities is preferred.
• CPR certification is required.
• Geriatric home health care experience is preferred.
• Self-starter with strong organizational skills.
• Compassionate, patient-centered care approach.
• Valid driver’s license required.
• Marketing experience is a plus.

Responsabilidades

• Conduct skilled nursing visits per patient care plan.
• Assist RN Case Manager in developing and updating plans of care.
• Administer nursing interventions per care plan.
• Monitor patient status and report changes to RN/physician.
• Educate patients and families about the plan of care and self-care.

Assisted Living Facility experience
