Job summary

Halcyon Home LLC is seeking a Home Health LVN PRN - Per Visit in Cypress, TX to support geriatric patients in Tomball/Spring and surrounding areas. Under RN supervision, you will provide skilled nursing care, patient education, and thorough documentation of care delivered.

Experience in home health is preferred; Homecare Homebase experience is a plus. Reliable transportation and a compassionate, team-oriented approach are essential for success.

Qualifications

Graduate of an accredited school of vocational/practical nursing.
Active state LVN license.
One year of LVN experience in a clinical care setting.
Home health experience preferred.
Homecare Homebase platform experience is a plus.

Responsibilities

Assist in identifying patient needs and plan of care under RN supervision.
Deliver nursing care as ordered and educate patients and caregivers.
Ensure infection control and documentation are maintained.

Essential Functions:

Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
Provides patient and caregiver education.
Provides care using infection control measures that protect both staff and patient.
Assures the continuity of care through delivery of quality patient care.
Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
Monitors assigned cases to ensure compliance with requirements of third-party payor.
Participates in the Agency’s QAPI program.
Participates in the Agency sponsored in-service trainings.
Demonstrates commitment, professional growth, and competency.
Promotes the Agency philosophy and administrative policies to ensure quality of care.
