Job summary

Halcyon Home LLC Cypress, Texas is seeking a Home Health Registered Nurse PRN Pay Per Visit to support our geriatric home health population in Tomball/Spring, TX and surrounding areas. You will perform initial and ongoing assessments, develop care plans, and coordinate with the care team to ensure quality, compassionate patient care.

The ideal candidate holds an active RN license, strong clinical and communication skills, and the ability to work independently.

Qualifications

Current RN license in good standing.
Experience in home health or related healthcare setting preferred.
Knowledge or experience with Homecare Homebase EMR is a plus.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage time effectively.

Responsibilities

Conduct initial and ongoing comprehensive assessments of patient needs.
Develop and monitor the plan of care for hospice patients.
Educate patients and families on disease, end-of-life care, nutrition and self-care.
Coordinate with the IDG and provide staff training as needed.
Assess acuity and determine service scope and frequency.
Provide clinical directions to Hospice Aide and LPN/LVN.
Document progress notes and ensure continuity of care.
Communicate changes in patient needs to physician and team.
Ensure compliance with third-party payor requirements.
