Halcyon Home in Tomball, TX is seeking a dedicated Home Health Registered Nurse PRN Pay Per Visit to support our geriatric home health patients in Tomball/Spring and surrounding areas. You will assess, plan, and deliver compassionate care in patients' homes, coordinating with families and the care team.

Formación

• Current RN license in good standing.
• Home health experience preferred; related healthcare background acceptable.
• Strong clinical assessment and communication abilities.

Responsabilidades

• Initial and ongoing assessments of patient needs in home settings.
• Develop and update plans of care to ensure quality and continuity.
• Educate patients and families about disease, care, and end-of-life considerations.
• Coordinate with the IDG and other health professionals to meet patient goals.
• Provide nursing procedures and document progress and outcomes.
