Job summary

Golden Valley Health Centers seeks a PACE Licensed Vocational Nurse in Merced, CA. Under the supervision of the supervising RN, you will assist in assessing and delivering skilled clinical services for participants in clinic, day center, and home settings.

The position runs Monday–Friday 8:00am–5:00pm at the Merced Center, with competitive benefits including health, dental, vision, and retirement options. Prior home or community health experience is valued.

Qualifications

• Valid CA LVN license required.
• Excellent written and verbal communication skills.
• Ability to work under stressful conditions.
• Creative, organized and detail-oriented work approach.

Responsibilities

• Under supervising RN, assist in assessing environment and needs of frail and elderly participants.
• Document information during participant encounters and review medical records.
• Assist with level of care tools, diagnoses, meds, ADLs, and IADLs as needed.
• Supervise Patient Care Assistants and ensure timely LOC recertifications.
