Job summary

Atrium is seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide skilled nursing care in patients' homes under RN supervision in the CenterWell network. The role emphasizes coordination with the care team, documentation, and adherence to state and Medicare standards.

The ideal candidate will have an LPN/LVN license, 2 years of relevant clinical experience, current CPR, and a valid driver’s license. Travel and light on-call duties may be required.

Qualifications

• Graduate of an accredited LPN/LVN program.
• Two years experience as an LPN/LVN in a clinical/home health or hospice setting.
• Current Nursing License in the practicing state.
• Must read, write and speak fluent English.

Responsibilities

• Assist in providing nursing services under RN supervision.
• Coordinate care with the interdisciplinary team and patient/family.
• Document outcomes and maintain accurate clinical records.
• Assist with admissions, discharges and transfers.
• Adhere to infection control measures (OSHA).
• Possibly perform on-call duties.
