Job summary

Rockville Center is seeking a motivated Licensed Practical Nurse (LPN) to join our skilled nursing team. The position requires NY LPN licensure, independence in evaluation and treatment, and a commitment to geriatric care.

Responsibilities include medication administration, vital sign monitoring, patient education, and collaboration with physicians and CNAs to ensure quality resident care. Evening and weekend shifts may be required.

Qualifications

• Current License-LPN (New York)
• Alternate Weekends

Responsibilities

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
