Job summary

GuideStar Eldercare in Indiana is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) to provide neurobehavioral health services to residents in long-term care facilities.

You will work with a multidisciplinary team to support elderly individuals with neurocognitive and psychiatric conditions, coordinate care with facility staff, PCPs, and families, and deliver evaluation and condition-specific therapy.

Qualifications

• Master’s degree in Mental Health Counseling or Social Work.
• Current LMHC or LCSW license in the state of application.
• Ability to work as part of an interdisciplinary treatment team.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy, or when indicated, insight-based therapy.
• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
