GuideStar Eldercare is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) to provide neurobehavioral health services to residents in long-term care facilities. You will collaborate with an interdisciplinary team to deliver holistic, continuity of care for elderly individuals with neurocognitive disorders and other conditions.

You will evaluate residents, deliver condition-specific therapy, and coordinate care plans with facilities, PCPs, and colleagues,

Qualificações

• Master’s degree in Mental Health Counseling or Social Work required.
• Current LMHC or LCSW license in the applying state.
• Ability to work in an interdisciplinary team with regional support.

Job Description:

As a Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker(LCSW) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.

What you will do:

• Provide evaluation
• Provide condition-specific support therapy, or when indicated, insight-based therapy
• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.

Required Qualifications:

• Ability to work as part of an interdisciplinary treatment team
• Good time management skills
• Able to function autonomously with regional Area Manager support
• Master’s degree in Mental Health Counseling or in Social Work
• Currently licensed as an LMHC or LCSW in the state in which you are applying

Preferred Skills:

• Passion for geriatrics
• Experience in inpatient settings such as extended care facilities
• Experience with brief bedside assessment screens (cognitive, mood, etc.)
• Motivated by productivity-based positions
• Ability to navigate and document on an Electronic Medical Record (EMR) system
