Job summary

LiveWell Homecare Agency, LLC seeks a Licensed Marriage and Family Therapist (LMFT) to provide in-person psychotherapy to elderly residents in skilled nursing, assisted living, and sub-acute facilities within a 45-minute radius.

You will assess, diagnose, and treat emotional and behavioral health issues, collaborate with facility staff and families, maintain accurate EHR documentation, and deliver care in a supportive environment with flexible full- or part-time schedules and a competitive

Qualifications

• Active, unrestricted LMFT license in the applicable state.
• Strong communication and organizational skills; proficient with electronic health records.
• Ability to provide in-person services within a 45-minute radius.
• Fully vaccinated where required.

Responsibilities

• Conduct individual and group therapy sessions, assessing, diagnosing, and treating emotional and behavioral health conditions such as depression, anxiety, and cognitive decline.
• Develop and implement evidence-based treatment plans in collaboration with facility staff, families, and interdisciplinary teams.
• Maintain accurate clinical documentation using a user-friendly EHR system, monitor patient progress, and adjust treatment as needed.
