Job summary

Strive Health seeks a Licensed Marriage and Family Therapist to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. You will conduct assessments, deliver individual and group therapy, and collaborate with an interdisciplinary team while ensuring regulatory compliance.

You will document sessions per Medicare and facility requirements, participate in care plan meetings, and support residents and families facing illness, placement, and end-of-life issues.

Qualifications

• Active LMFT license.
• Master's in counseling or related field.
• Enrolled or eligible for enrollment as a Medicare Part B provider.
• Experience working with geriatric populations or in long-term care preferred.
• Strong interpersonal, documentation, and communication skills.
• Ability to work independently within a collaborative, team-based environment.

Responsibilities

• Conduct initial diagnostic assessments and develop individualized treatment plans.
• Provide psychotherapy services tailored to resident needs.
• Deliver individual, group, and family counseling focused on depression, anxiety, grief, adjustment, and behavioral health issues.
• Document sessions in compliance with Medicare and facility requirements.
• Participate in care plan meetings and interdisciplinary team discussions.
• Support residents and families in coping with illness, placement, and end-of-life issues.
• Collaborate with Strive psychiatric nurse practitioners and facility staff to ensure coordinated care.
• Educate facility staff on recognizing psychosocial needs and appropriate referral processes.
