Qualifications

• Active, unrestricted LMFT license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience in geriatric LTC settings preferred.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, grief, and behavioral health needs.
• Coordinate with nursing, social services, rehab, and medical teams.
• Maintain documentation using GNT and comply with HIPAA.
