Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Clinical Social Worker (LCSW) to join our integrated clinical team onsite at local long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support to residents.

Qualifications

• Active, unrestricted LCSW license.
• Experience delivering therapy in geriatric settings.
• Ability to work with interdisciplinary teams.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct psychosocial assessments and treatment planning.
• Collaborate with nursing and medical staff to coordinate care.
• Maintain compliant clinical documentation using the GNT.
